Spontaneous resolution of new coronary artery aneurysm following guideline-directed medical therapy after drug-eluting stent implantation

چکیده

Here, we present a case of 56-year-old man with acute myocardial infarction. The patient underwent percutaneous coronary intervention (PCI) at the left main bifurcation and mid-left anterior descending artery using drugeluting stents. Four months after PCI, was readmitted for cardiac arrest. Coronary angiography (CAG) revealed stent thrombosis in main-toproximal in-stent restenosis circumflex artery. We performed balloon angioplasty to proximal stents; subsequently, blood flow fully restored. However, contrast agent extravasation observed outside mid-portion stent, indicating presence aneurysm (CAA) stent. After guideline-directed medical therapy dual antiplatelet agents high-intensity statins, follow-up CAG near-resolution CAA, absence stenotic lesions, good flow.

Coronary aneurysm after drug-eluting stent implantation.

The use of drug-eluting stents aiming at by-pass the disadvantage of stainless steel stents have been associated to late thrombosis after withdrawal of anti-platelet agents. We report a case with another complication, the development of a coronary aneurysm in the stent area more than three years after index procedure. Late chronic local inflammatory responses may be responsible for the weakenin...

Coronary Aneurysm Occurring Late after Drug-Eluting Stent Implantation

Drug-eluting stents may affect the normal healing process of the vessel wall and the remodeling process and may lead to late stent malapposition (LSM). The known incidence of this phenomen originates from short-term angiographic follow-up studies. We describe a case report of very late stent malapposition and marked positive vessel remodeling 3 years after sirolimus-eluting coronary stent impla...

Coronary artery aneurysms after drug-eluting stent implantation.

Drug-eluting stents (DES), which locally elute antiproliferative drugs, can dramatically inhibit neointimal growth. However, several pathological studies have indicated that DES may delay healing after vascular injury, and DES implantation may be theoretically associated with a risk of coronary artery aneurysm formation. Coronary aneurysms have been reported from 3 days to up to 4 years after D...

Giant Coronary Artery Aneurysm after Drug-Eluting Stent (DES) Implantation: Therapeutic Challenges and Management

Introduction of Drug-Eluting Stent (DES) proved to be an important step forward in reducing rates of restenosis and target lesion revascularization after percutaneous coronary intervention, but at the same time potentially causing serious complications. One of the rare complications of DES is coronary artery aneurysm. Coronary Artery Aneurysm (CAA), defined as localized dilatation of the corona...
